Problem: Accountable care organizations (ACOs) lose revenue when their patients obtain care outside of their network. The patient can go outside of the ACO for a number of reasons. Specifically, physicians may unknowingly refer the patient outside of the network. Alternatively, the ACO may have gaps in the types of services they offer and need to hire additional specialists.

Solution: We developed a web based application for physician executives based on the organization's claims data. The application helped executives understand what factors were driving their leakage. It provided actionable information, such as where there were gaps in their networks so they could attempt to expand their services. Additional, it provided the names of practices and physicians that had the largest amount of leakage so that the ACO could provide education and resources on how to improve referral patterns.

This idea came from the winner of our company's annual Innovator of the Year competition.

Problem: Case managers and social workers do not have an adequate search tool to help them find local social programs for their patients. We heard that they were using a number of subpar solutions to keep track of resources, such as personal excel documents, e-mail, browser bookmarks and paper.

Solution: We developed a crowd-sourced search tool that allowed users to find resources in their area, read and write reviews of the organization and save their favorites. The data came from a number of sources, including users, the company SharePoint and 3rd party APIs. We created an admin interface to allow admins to review and approve submissions of new organization listings. We also created a small toolset to help admins manage the large dataset and avoid duplicates or incorrect listings.

This idea came from the winner of our company's annual Innovator of the Year competition.

Problem: Individuals or caregiver who are dealing with a difficult diagnosis want to connect with others who know what they are going through. Through this connection, they can share their experiences and provide insight and advice on how to navigate the diagnosis and health care system.

Solution: CarePal is a mobile optimized web application targeted towards to women with breast cancer. It allows women who have been newly diagnosed to connect with survivors in order to provide a mentoring relationship. Women are matched based on socio demographics, diagnosis and topics of interest. The application was designed in a way that it could be rolled out to other conditions, such as caregivers of family members with Autism and Alzheimer's.

Problem: Our team had video content for several ideas that we wanted to test. Our ideas included mindfulness for back pain and tai chi for falls prevention. We needed an easy way to spin up new pilots with applications to support them and gather outcomes data.

Solution: We created a content delivery framework that could be stood up in 3 days for any pilot as long as there was video content available. We built in weekly surveys to collect health data from the user in order to measure outcomes and success. The framework is available for web, mobile and tablet implementation.

Our team was responsible for researching new ideas that could potentially reduce patient costs, improve outcomes or improve the patient experience. One of our team members had a son with cancer and though his experience, we identified several areas where technology could help alleviate the burden of a cancer diagnosis. These areas included social, financial, clinical and emotional challenges.

We decided to focus on breast cancer for our research and prototype. We interviewed several women with breast cancer, ranging from newly diagnosed to survivors for 10+ years. We synthesized our findings into a set of problems statements and prioritized them based on the severity of the need and our ability to solve the problem. We conducted a design studio as a team to sketch out what a solution might look like for our top problems.

As a result, we came up with a modular care management app that could be customized to the patient's diagnosis and treatment plan. The app would help keep the patient on track with all of the tasks they needed to complete as well as manage their expectations about what was ahead. Not only did this tool help empower the patient to proactively manage the healthcare system, but it also provided support and guidance through a coaching function. At any time, a patient could message their care management coach for help. We heard from several people that they had enough clinical support and did not want additional phone calls from their health insurance, so this app would be a perfect way to provide passive but meaningful support.
