      <image:caption>role: head of product | product: Smart Networks Problem: Accountable care organizations (ACOs) lose revenue when their patients obtain care outside of their network. The patient can go outside of the ACO for a number of reasons. Specifically, physicians may unknowingly refer the patient outside of the network. Alternatively, the ACO may have gaps in the types of services they offer and need to hire additional specialists. Solution: We developed a web based application for physician executives based on the organization's claims data. The application helped executives understand what factors were driving their leakage. It provided actionable information, such as where there were gaps in their networks so they could attempt to expand their services. Additional, it provided the names of practices and physicians that had the largest amount of leakage so that the ACO could provide education and resources on how to improve referral patterns.</image:caption>
